Unix, it is for the most part an unexcpeted appearance. The process Ive seen begins after about 10 - 15 mins of exploration and battle, the environment turns dark, music changes, and a Sorceress flys in and you have to follow her. I think location wise, that's probably the random part. There is a demo on youtube, can't remember which video number or time stamp it was, but that could also be helpful.
